Health Inequalities Team (HI)

Lead by our Health and Wellbeing Coach and Specialist Clinical Nurse, the Health Inequalities Team identify and targets groups within our patient population that experience unequal access to healthcare such as those that are rurally isolated, living with long term conditions or mental health problems or experiencing homelessness.
They currently provide a number of clinics from our mobile clinical unit, and details of clinics accessible to patients is over on our clinics page.
