The number of services offered by Malling PCN is increasing month by month. Below are some of the main things we're doing right now.

PCN Services

Cancer Care

Our Cancer Care Coordinator gets in touch with newly diagnosed cancer patients to provide an array of support to patients and their families.

This may include help with completing applications for a blue badge or financial benefits, connecting patients to counselling or support groups or inviting them along to PCN run coffee mornings for peer support. Our cancer care coordinator keeps in touch with patients to support them through treatment and can advocate and help them navigate each step.

Children and Young People with Neurodiversity

Our Children’s Health & Wellbeing Navigators support children, young people and their families around navigating the neurodiversity pathway, whether that be the NHS or Right to Choose Pathway as well as signposting and linking individuals in with services that can support them with any challenges they may be facing.

They can support patients to communicate with community paediatrics, children's therapies, school issues or referral issues and offer advice and links to tools that may help in the family home. The team run family support sessions throughout the year, and a list of these can be found on our services page.

Children’s Mental Health Service

Our Children and Young People’s mental health service is accessible through your practice reception teams.

If you have more urgent Mental Health concerns about a young person, please follow the following advice: - NHS 111 Option 2 and Mental Health Direct Crisis Line if you are experiencing something that makes you feel unsafe, distressed, or worried about your mental health, dial 111 Option 2 for Mental Health, or call Mental Health Direct on 0800 995 1000.

Covid Vaccinations

Our vaccination hub is based at Avicenna Medical Centre, Oxley Shaw Lane, Leybourne.

The Malling PCN COVID-19 vaccination programme is very fortunate to have help and support from our local community, including Malling Rotary, Leybourne Parish Council, The Old Rectory Pub, and a band of fantastic vaccinators, many of whom are healthcare professionals living in our area.

Our MCU is often out in the community offering additional access to vaccinations for our local community and the wider West Kent area. For more information on when and how you can book an appointment, see our Covid Vaccination Clinic section

Learning Disabilities

Our learning disability care coordinator supports our patients with learning disabilities, to ensure that they have access to appropriate healthcare, take advantage of all screening and health promotion opportunities available to them, and promote initiatives such as the hospital passport.

They will invite everyone on the learning disability register for their annual health check and can provide regular well-being check ins, support with applying for PIP, employment support, advocacy and lots more.

Menopause Cafe's

At Malling PCN, we recognise how challenging the menopause can be for a woman's overall health and wellbeing. To support our patients through this stage of life, one of our Care Coordinators is running a series of sessions throughout the year. These community-based events feature a range of clinical and holistic guest speakers, offering advice, support, and practical tools to help empower women during the menopause. The sessions also provide a safe, welcoming space for open discussion and shared experiences.

Mobile Clinics (MCU)

Mobile Clinics (MCU)

A former ambulance owned and converted by Malling Primary Care Network is used out in the community to offer a variety of mobile health clinics.

Clinics are run by the Health Inequalities Team and include Men’s Wellman Checks, Health Checks for Unpaid Carers and Pop-Up Health Checks open to all.

Neighbourhood Care Centre (NCC)

The Neighbourhood Care Centre (NCC) is a central hub located in Larkfield which houses 2 clinical rooms used primarily by the Clinical Assessment Team (CAT) for the Neighbourhood Care Centre (NCC) Same Day Appointments and Extended Access (evening) appointments, but is also utilised for some additional clinics, including weekend Spirometry and Physiotherapy clinics.

It also acts as an administrative hub for multiple teams and a location for training and meetings for our staff.

Social Prescribing

Many things that affect our health and wellbeing can’t be treated by doctors or medicine alone. Social prescribing connects people to non-medical community-based support, including activities and services that meet practical, social, and emotional needs that affect their health and wellbeing. This includes connecting people to statutory services for example housing, financial and welfare advice.

Any patient within the PCN can self-refer to the Social Prescriber via their practice reception or by contacting Involve Kent.

Appointments can be via telephone or face to face and the initial consultation will take time to talk through everything that may be going on and work out what is important to you.

Home Visiting Service

The Clinical Assessment Team attend housebound patients on behalf of the GPs, assessing patients face to face, enabling the GPs to make decisions for their care. By utilising the home visiting service, GPs can spend more time at surgeries, seeing additional patients, whilst still ensuring the housebound patients in our community receive a great standard of care.

Our Clinics

Neighbourhood Care Center Same Day Appointments

The Neighbourhood Care Centre provides a same-day urgent care service across the practices, helping to relieve pressure on GP practices and providing additional same day appointments for the PCN.

This is run by Advanced Clinical Practitioner Paramedics and First Contact Paramedics who can see a wide range of same day minor ailments.

Men’s Wellness Checks

Accessible to all males aged 50+ and offered at a variety of locations out in the community, such as Leybourne Chase, Snodland Community Centre and Ditton Community Centre.

The screening will assess lifestyle and health habits, with services including mole checks for skin cancer, urine tests, PSA blood tests, FIT kit access, and referrals for weight loss, smoking cessation, or further GP investigation when necessary For Men Wellness Checks for patients of our practices please call: 0333 332 6700 Option 4.

Carer’s Health Checks

Accessible to all unpaid carers providing support to a loved one and offered at a variety of locations out in the community such as Leybourne Chase, Snodland Community Centre and Ditton Community Centre.

You do not need to be a registered carer with your GP surgery or elsewhere to book or attend an appointment, but we encourage you to register with your surgery so that you receive any information about carer specific health events.

Health checks include checks for BMI, blood pressure, pulse, sugar levels and cholesterol and can provide practical support to manage your physical and mental health and wellbeing.

Pop-Up Community Health Checks

Health checks include checks for BMI, blood pressure, pulse, sugar levels and cholesterol and can provide practical support to manage your physical and mental health and wellbeing.

Pop-Up Community Health Checks do not need to be booked in advance. If you are a patient at one of our practices, simply call 0333 332 6700 Option 4 for more information on dates and locations.

Spirometry Appointments

Our ARTP trained clinicians provide the Diagnostic Spirometry Service for patients within our PCN, and across the wider West Kent area. This test is used to check patient’s lung function, and to help diagnose Asthma and COPD.
